We believe that a multidisciplinary approach is best way to give our patients the necessary tools they need to achieve the highest quality of life possible. One group of professionals cannot address every issue in a complex case. We have a team of professionals who are prepared to meet the individual needs of our patients.
Registered nurses (RNs) are the core of our company. These experienced professionals manage their patient’s total health status, referring to other team members when appropriate. Our RNs have excellent assessment skills, and are able to identify the early signs of a potentially dangerous situation. Our RNs educate their patients on their existing disease processes, medications, and any lifestyle changes.
Texas Cardiac Care provides a full range of therapy services, including: physical therapy (PT), occupational therapy (OT), and speech therapy (ST). If needed, our therapists can be involved in the evaluation and management of any disease process that limits a patient’s ability to perform daily activities. Patients will receive individual therapy programs designed to meet their individual needs.
Our home health aides (HHAs) are certified by the Texas Department of Aging and Disability Services (DADS). They can assist patients with everyday activities such as personal hygiene and light housekeeping. Our HHAs provide a safer environment by following a specific care plan developed by a case manager.
Texas Cardiac Care can provide a licensed clinical social worker (LCSW) to assist patients with long term care placement, financial concerns, or referrals to community resources.
